What you will do:
- Utilize your community rehab experience to provide exceptional care to our patients.
- Employ your excellent assessment and rehab skills to assist patients with cognitive and physical difficulties.
- Conduct complex manual handling and assess patients within their homes to ensure safety and progress.
- Issue necessary equipment tailored to individual needs and create comprehensive care plans.
- Develop home exercise plans to support patient rehabilitation and independence.
To excel in this role, you should hold HCPC registration and bring at least two years of experience in a similar speciality, particularly in elderly care. While having a car for travelling to different home visits is preferable, you will also find efficient transport links across the area.
