Key Responsibilities

Undertake timely physiotherapy assessments on Ward 7 to determine patients’ discharge readiness

Plan and deliver appropriate physiotherapy interventions to support safe and effective discharge

Participate in parallel planning to ensure continuity of care and patient flow

Develop, implement, and review individualised rehabilitation and exercise programmes

Liaise closely with hospital teams, community services, and multidisciplinary colleagues

Contribute to efficient patient flow and discharge processes

Maintain accurate and timely clinical documentation in line with professional standards
Essential Requirements

HCPC registration as a Physiotherapist

Proven experience working within an acute hospital setting

Ability to work independently and as part of a multidisciplinary team

Strong communication and organisational skills

Full UK driving licence to support community liaison where required
