Salary: Up to £29 per hour, Full-time

What you will do:
- Assess and treat adult patients with neurological conditions, with a focus on intervention for acquired communication and swallowing difficulties in the community setting
- Provide support to patients in their homes, care homes, and other community settings alongside Sutton GPs
- Manage patients with conditions like stroke, head injury, Parkinson’s disease, multiple sclerosis, and other progressive conditions
- Work autonomously in managing dysphagia, ensuring comprehensive care delivery
- Collaborate effectively within the Multidisciplinary Team (MDT) to facilitate holistic patient care
- Complete community visits around the borough of Sutton, ensuring the best possible support for patients
- Regularly document assessments and interventions using EMIS, provided you have a NHS SMART card
Essential requirements:
- Proven experience working with patients with neurological conditions
- Experience in rehabilitation and managing long-term conditions
- Dysphagia trained with the ability to work autonomously
- Community experience with adults and older adults
- Proficiency in MDT working
- Must have a car and a valid driving licence
Desirable:
- EMIS trained
- Able to claim mileage, although parking permits are not available
