What you will do:
- Deliver comprehensive assessments, diagnosis, and treatment plans for adult patients
- Work collaboratively within a multidisciplinary team to provide outstanding community care
- Implement patient-centred therapy programmes, ensuring each individual’s needs are met
- Provide guidance and support to patients, families, and caregivers
- Maintain accurate documentation and report writing, ensuring the highest standards of care
- Engage in regular team meetings and case discussions for robust peer support
