What you will do:
- Provide high-quality Speech and Language Therapy services to adults with neuro conditions, focusing on stroke, general medical issues, and elderly frailty.
- Deliver services in patients’ homes, at clinic sites, and in nursing homes across the community.
- Conduct comprehensive assessments to develop tailored treatment plans for each individual’s needs.
- Collaborate with a multidisciplinary team to ensure holistic care, and contribute to team meetings and case discussions.
- Deliver training and support to families and other healthcare professionals to maximise patient outcomes.
