What you will do:
- Work as part of the multi-disciplinary memory assessment service team, independently assessing and planning towards diagnosis.
- Utilise your specialist knowledge of dementia strategies and interventions to provide effective treatment.
- Refer patients to other in-service agencies or external services as necessary, ensuring comprehensive care delivery.
- Confidently identify risk factors and take appropriate actions to mitigate them.
- Facilitate seamless communication between GPs, LCFT services, and non-statutory services to uphold high standards of patient care.
- Offer mentoring and clinical supervision for other staff and trainees.
- Embody and actively promote the Trust values within the service, adhering to NMC and HPC codes of conduct.
- Assist the team manager in delivering effective leadership, ensuring that patient and carer assessments, formulations, and care planning are consistently high in quality.
