What you will do:
- Collaborate with the Integrated Discharge Team to facilitate safe and effective patient discharge planning from hospital to home or other care settings.
- Assess and develop individualised physiotherapy care plans to promote patient recovery and independence.
- Conduct comprehensive patient assessments, identifying needs and setting goals to enhance mobility and function.
- Provide guidance and support to patients and their families on rehabilitation exercises and strategies.
- Maintain accurate and up-to-date patient records and documentation in line with professional standards.
To thrive in this role, you must hold a current HCPC registration, ensuring that you meet the professional standards required to practice physiotherapy in the UK.
