What you will do:
- Undertake client assessments using standardised assessment tools and the single assessment process
- Conduct holistic assessments and utilise standardised falls assessment tools to develop a multidisciplinary care plan
- Maintain effective multidisciplinary communications to ensure treatment goals are achieved and a high-quality service is delivered to clients
Requirements:
- HCPC registration is essential
- Minimum of 2 years relevant experience in physiotherapy
- A car driver is preferable to facilitate travel across multiple sites
- Strong communication and teamwork skills are a must
