Key Responsibilities:
- Conduct assessments and develop tailored intervention plans for elderly patients with mental health needs.
- Collaborate within a multidisciplinary team to deliver comprehensive care and support.
- Implement therapeutic activities to improve patients’ well-being and independence.
- Educate and advise patients, families, and carers to promote positive health outcomes.
- Ensure accurate and timely documentation to maintain high standards of care.
Preferred Experience and Requirements:
- Experience in mental health, particularly with older adults, is advantageous.
- Previous Band 6 experience is highly desirable.
- HCPC registration is essential.
- A valid driving licence is preferable to support client outreach.
