What you will do:
- Facilitate and manage the discharge of patients from acute and community hospitals, ensuring a smooth transition for each individual.
- Collaborate effectively with teams of Occupational Therapists, Physiotherapists, Therapy Assistants, and Reablement Workers to deliver first-class care.
- Conduct assessments to determine each patient’s needs and develop tailored care plans.
- Provide comprehensive occupational therapy interventions to enable patients to return safely to their homes.
- Liaise with healthcare professionals, patients, and their families to ensure clear communication and a coordinated approach to patient care.
Requirements:
- HCPC Registration: Ensure you are registered and meet all necessary professional standards.
- Full UK Driving Licence: Essential for travelling between various locations across Bath.
