What you will do:

– Conduct comprehensive assessments of patients’ physical, psychological, and social needs within their homes or community settings.
– Develop, implement, and review personalised intervention plans to support patients’ rehabilitation and daily living skills.
– Collaborate closely with other healthcare professionals, social services, and community agencies to ensure integrated care delivery.
– Support patients in regaining independence following hospital discharge or managing long-term conditions.
– Provide education and advice to patients, families, and carers to promote health and wellbeing.
– Maintain accurate and timely clinical records in line with professional standards and organisational policies.
– Participate in team meetings, case conferences, and professional development activities.

Person Specification:

– Registered Occupational Therapist with the Health and Care Professions Council (HCPC).
– Proven experience working in community or rehabilitation settings, ideally at Band 6 level or equivalent.
– Strong clinical reasoning and assessment skills.
– Excellent communication and interpersonal skills to engage effectively with patients and multidisciplinary teams.
– Ability to work independently and manage a varied caseload.
– Commitment to continuous professional development and evidence-based practice.
