Band 6 Occupational Therapist – Community Team in Welwyn Garden City, earning £26 per hour, Hourly. The role starts ASAP and is ongoing. Based in the charming Welwyn Garden City, you will work across multiple sites assisting with hospital discharges and providing hands-on patient care.

What you will do:
- Conduct assessments of patients within 72 hours after their discharge from hospital admissions.
- Coordinate with various agencies and provide necessary equipment from your car to aid patient recovery.
- Be punctual with your attendance, starting at base by 8am and concluding your duties by 4pm at base or leaving the patient’s property.
- Efficiently complete patient notes on a laptop, ideally during patient interaction for accuracy and efficiency.
- Possibly work during the weekends, providing you with opportunities to earn even more.
Requirements:
- HCPC-registered Occupational Therapist.
- Minimum 2 years of experience in a relevant community or hospital discharge setting.
- Car driver with access to own vehicle – preferred for this role due to travel requirements.
- Proficiency using SystmOne electronic records system.
