What you will do:
- Collaborate within an integrated network team to facilitate patients’ smooth discharge from hospital settings
- Implement strategies to prevent patient admissions through effective community-based interventions
- Undertake tailored therapeutic interventions aimed at rehabilitation and recovery within patients’ homes
- Plan and deliver therapy as part of Pathway 1 and Urgent Community response initiatives
- Utilise your skills and expertise to support patients in their journey to independence and improved well-being
This role requires you to be HCPC registered and proficient in driving, as the nature of the job involves travel within the community.
