What you will do:
- Manage a defined caseload within your specialist area, using evidence-based and client-centred principles
- Assess, plan, implement, and evaluate the outcomes of interventions in the community, ensuring records are maintained to an agreed standard
- Organise your workload efficiently with regard to clinical priorities and time management
- Keep accurate records and provide thorough reports as needed
- Collaborate with clients, carers, and the multidisciplinary team to identify, set, and achieve relevant goals and interventions
