Sanctuary Social Care is currently looking for an experienced and dedicated Mental Health Social Worker to work full-time in Croydon, within the Assessment & Liaison Team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Mental Health Social Worker job is £25.44 per hour (Assignment rate) and is an initial 3-month contract.
Duties of the Mental Health Social Worker:
- To oversee and provide assessment, planning, of care to a group of service users.
- Ensuring records associated with this process are maintained.
- To work effectively within a multi-disciplinary team and across other agencies.
- To provide a high quality of care to individuals with complex bio psychosocial needs.
- Carry out procedures to a standard that ensures safe and effective care, and complies with their relevant professional code of conduct.
- Clinically supervise and appraise junior staff and students.
- Facilitate user involvement in all aspects of service delivery.
- To formulate needs assessments for service users requiring commissioned services and to monitor care packages as directed.
- To ensure that risk assessments are completed in full, documented and actioned.
- To provide continuing assessment and follow up of the individualised care plan, liaising with appropriate stakeholders.
Requirements of the Mental Health Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
