Sanctuary Social Care is currently looking for an experienced and dedicated Band 6 Hospital Social Worker to work full-time in Manchester. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults' Services and be registered with Social Work England.
The pay rate for this locum Social Worker job is £25 per hour (Assignment rate) and is an initial 3 month contract.
Duties:
- Work as part of the family support team to offer palliative care support and advice to people with terminal illnesses.
- Work in partnership with five Local Authorities and CCG's to enable timely discharges.
- Facilitated and Chaired complex discharge planning meetings with patients, families and the multidisciplinary team.
- Advised and signpost parents to the relevant support child support and bereavement agencies.
- Facilitate DS1500 applications
- Coordinate CHC Fast Track applications.
- Attend daily ward rounds with the multidisciplinary team.
- Refer safeguarding concerns to the relevant Local Authority and support Safeguarding investigations.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant senior level frontline experience within Adults' Services
