What you will do:
- Plan, implement, review, and improve interventions to meet the identified needs of individuals and effectively manage their inherent risks.
- Contribute to case conferences and reviews, supporting service users and carers to engage actively with services and play a part in their own care.
- Accept ongoing accountability for service users, manage a delegated caseload of complex cases, and provide supervision and mentorship to junior staff and learners in the provision of direct and indirect nursing care.
- Provide duty support to patients within the CMHT, either via telephone or face-to-face, ensuring their needs and risks are assessed and managed effectively.
- Develop comprehensive risk management plans and escalate care where necessary to ensure the best outcomes for patients.
