What you will do:
- Plan, implement, review, and improve interventions tailored to individuals’ needs while effectively managing risk.
- Actively contribute to case conferences and reviews, encouraging involvement from service users and carers.
- Maintain accountability for service users by managing a delegated caseload, focusing on complex cases, and supervising junior staff as needed.
- Provide duty cover, offering support to CMHT patients either by phone or face-to-face.
- Conduct comprehensive risk assessments and create management plans, escalating care when necessary.
