Sanctuary Health is currently looking for an experienced and dedicated Band 6 Community Psychiatric Nurse to work full-time based in Chelsea. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.

The pay rate for this locum Community Psychiatric Nurse role is £25 - £27 per hour (Assignment rate) and is an initial 3-month contract.

Job Summary:

  • This post is with the Community Living Well service in which the primary care liaison nurses are an integral part of the service. This service is based on the ethos of providing a vibrant, resilient and community-integrated model of care, the Community Living Well (CLW) service, that better secures the mental, physical and social health of those with common complex and long-term mental health needs, supporting and enhancing the role of the GP as the accountable clinician, and that provides a seamless interface with secondary acute mental health services.
  • The community living Well (CLW) service is an integrated, tiered model of care bringing together a co-ordinated range of wellbeing and clinical services including existing clinical wellbeing services proved by CNWL and the voluntary service together with resources in the wider RBKC and North Westminster Boroughs. You will be part of a well-established PCLN team. This service will aim to meet the needs of those with stable complex mental illness, and Common Complex Mental illness.
  • In this role you will provide structured assessments, including risk assessment and risk management plans, care planning and time limited interventions for patients referred to the team, liaising closely assigned surgeries within the 45 West London CCG G.P. surgeries.
  • It is expected that in this role you will take a proactive and vigilant approach in the community, through earlier intervention and promoting enhanced self-care, leading to better health of those with mental health difficulties, which in turn will lead to fewer referrals to secondary care while facilitating transfer to primary care, "step-downs".

Requirements of the Community Psychiatric Nurse:

  • Valid NMC registration under Mental Health
  • Experience working within a similar role would be highly desirable
  • A degree or equivalent within Mental Health Nursing (RMN trained)
