What you will do:
- Conduct comprehensive nursing assessments collaboratively with service users and their families or carers
- Develop personalised care plans aimed at promoting well-being and community inclusion
- Deliver evidence-based interventions following NICE guidelines to support recovery and prevent relapse
- Regularly review care plans and outcomes, making necessary adjustments for optimum results
- Provide specialist nursing advice and consultation to multi-agency services
