Role Responsibilities

Deliver day-to-day pharmacy technician services on Welney Ward, supporting inpatient care

Work collaboratively with pharmacists to provide specialist clinical pharmacy support

Conduct medicines reconciliation for newly admitted patients and assess patient-owned medication
-
Accurately document issues and coordinate with medical teams to resolve medication queries

Order medications and maintain ward stock to ensure availability

Attend weekly MDT meetings to assist with patient care planning and discharge coordination

Promote patient independence by supporting self-administration of medicines

Liaise with community pharmacies to ensure continuity of care at admission and discharge
Essential Requirements

Registration with the Health and Care Professions Council (HCPC)

Valid UK driving licence

Proven experience within a hospital pharmacy or clinical setting

Strong communication and organisational skills

Ability to work independently and as part of a multidisciplinary team
