Sanctuary Health is currently looking for a Band 6 Care Co-Ordinators - Community Psychiatric Nurse to work within an Older Adults unit based in High Wycombe.
The pay rate for this full-time role is £28 - £30 per hour (Assignment rate) and is an initial 3-month contract.
Job Summary:
- Care Co-ordination for Older Adults in the community with dementia and functional disorders.
- Memory Nurse - working in a memory assessment service. Will need to understand memory assessment, diagnosis and relevant medication
Requirements of the Community Psychiatric Nurse:
- Valid NMC registration under Mental Health.
- Experience working within a similar role would be highly desirable.
- A degree or equivalent within Mental Health Nursing (RMN trained).
- Car driver with access to a vehicle and appropriate car insurance
- Experience working with Older Adults in community settings
- Experience of care co-ordination, carer assessment, mental health assessment, safeguarding and suicide prevention
- CPNs - experience of depot and medicine management
- Dementia diagnosis experience
