Sanctuary Health is currently looking for an experienced and dedicated Band 6 Care Coordinator (CPN) to work full-time within a Treatment Team based in Abingdon. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC.
The pay rate for this Care Coordinator role is £25 - £30 per hour (LTD Company Equivalent) and is a 3-month contract.
Summary:
- They will have a caseload of around 20 patients who are subject the Care Programme Approach (CPA) framework and will be expected to carry out interventions to meet their needs and support them to achieve their goals.
- The team is supported by the team manager, the deputy manager and the consultant psychiatrist, and the MDT relies heavily on team work from the full MDT of care co-ordinators, support worker, psychologist and admin team.
Requirements of the Care Coordinator:
- Valid NMC registration under Mental Health
- Experience working within a similar role would be highly desirable
- A degree or equivalent within Mental Health Nursing (RMN trained)
- Must have a Full drivers licence with access to a car for business use
- Must have experience of working in community mental health settings with patients with severe and enduring mental illness.
