What you will do:
- Conduct comprehensive assessments of patients in their homes or suitable community settings
- Develop and implement tailored rehabilitation programs focusing on stroke recovery
- Work collaboratively with multidisciplinary teams to ensure coordinated patient care
- Engage with patients and their families to set and achieve rehabilitative goals
- Maintain accurate records and documentation in line with NHS standards
- Use your car to travel efficiently between appointments and manage your schedule effectively
This role requires a car driver due to the community nature of the work, ensuring fast and flexible access to clients. Applicants must have a minimum of 1 year’s UK NHS experience and be HCPC registered to apply.
