Sanctuary Personnel, an award-winning and renowned Recruitment Agency is looking for a
Band 5/6 Urgent Community Response Team Physiotherapist/Occupational Therapist to work full-time based in Epping/Harrow.
The pay rate for this locum Physiotherapist/Occupational Therapist job is £25-27 per hour (Assignment rate) and is a 3-month contract.
Main duties:
- Work within the Urgent Community Response Team (UCR) as part of the wider Integrated Care Team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service
- Carry out initial assessments and planning care for patients on the Urgent Community Response Service who are in crisis in need of rehabilitation and reablement following discharge from hospital or to prevent hospital admission
- Work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need
- Expected to work multi professionally, support other team members’ roles,
- Contribute to the future development of the service providing professional advice and appropriate training to others in the multi-disciplinary service
Requirements of the Physiotherapist/Occupational Therapist:
- Degree or equivalent within Occupational Therapy/Physiotherapy
- HCPC registered as an Occupational Therapist/Physiotherapist
- Recent post qualifying experience working as an Occupational Therapist/Physiotherapist
