Sanctuary Social Care is currently looking for an Approved Mental Health Practitioner (Social Worker) to fill a full-time role based in Tower Hamlets, London. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adults services and be registered with the HCPC.

The pay rate for this Approved Mental Health Practitioner role is £36 per hour (LTD Company Equivalent) and is an initial 3-month contract.

Main Purpose of the Job:

  • Working in integrated/partnership within the Community Mental Health Team/East London NHS Trust.
  • Provision of a social work service within the framework of the Care Programme Approach & Care Act plus other appropriate legislative requirements.
  • Working with individual service users in hospital and in the community including placements providing advice, assistance, assessment, review, or other forms of social work assistance within HCPC parameters with the aim of enhancing the outcomes for service users and carers.

Duties and Responsibilities:

  • Comply with the statutory requirements of the relevant childcare, mental health or Care Act legislation and in line with the requirements of the Council's policies and procedures.
  • Respond to requests for assistance, taking sufficient basic information about the needs in question to be able to determine the level and type of assessment required as per Care Act and other legislative requirements.
  • Maintain case records and fulfil procedures and statutory duties in line with national and local policies, practices and procedures.
  • Inform the line manager of specific needs that may be unmet because of the unavailability of services.
  • To utilise community resources to meet such needs as appropriate.
  • Work within budgetary limits laid down by Council policy and operate within the Financial Regulations and Standing Orders of the Council.
  • Carry out assessment of needs, and sub-contract parts of such assessments where they are of a specialist nature within the process of the Care Programme Approach and Care Act.
  • Ensure that assessments pay particular attention to individual strengths, outcomes and the recovery principles.
  • Ensure that assessments take into account needs in all relevant user categories.
  • Negotiate with Service users, carers and others concerning the methods to meet the needs and thus outcomes identified by each assessment and service user/carer.
  • Ensure that care plans include clear statements of agreed outcomes. Service or resource to be provided via the care plan and in line with Council requirements.
  • Assist users and carers to play a full part in care planning by extending to them advice, support and guidance, including advocacy, and arranging where necessary access to translation, interpretation services.

Requirements:

  • A Social Work qualification
  • Current HCPC Registration
  • Significant senior level frontline experience within Adult services.
  • Approved Mental Health Professional or will undertake requisite training.
  • Mental Health Experience (including work with mentally ill clients and their carers)
