Sanctuary Personnel, an innovative and committed recruitment agency has jobs based in Cornwall for Band 7 Advanced Community Practitioners to work within the Ageing Wellbeing Team.

The pay rates for these locum Advanced Community Practitioner jobs are £29.00 - £30.00 per hour (Assignment rate) and are initial 3-month contracts.

These roles will be based at multiple sites.

Duties:

  • Supporting the design and development of a community based urgent care response as part of the Ageing Well Programme which will align with the Embrace Community Intervention Offer (CIO)
  • Test new models of care delivery and advanced practice roles, this will necessitate a flexible and responsive individual who is confident to adapt and work with autonomy and ambiguity
  • Consult, assess, and treat patients who are referred with undifferentiated, unpredictable, varied and complex needs ensuring holistic assessment, prompt treatment and community care pathway planning is undertaken to directly prevent unnecessary acute hospital admission

Requirements of the Advanced Community Practitioners:

  • Degree or equivalent within Occupational Therapy
  • HCPC registered as an Occupational Therapist
  • Recent NHS experience either pre or post qualification
  • An innovative and tenacious clinician, with a proven track record or recognised potential for embracing and leading change
  • Vision and enthusiasm to help codesign and transform our evolving community services within Cornwall
  • Be able to undertake advanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review
  • Be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission
  • Be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards
  • Car driver
