What You Will Do
- Assess Needs – Conduct comprehensive assessments to identify the needs of adults and develop personalised care plans.
- Coordinate Care – Collaborate with multidisciplinary teams to implement and monitor effective care and support plans.
- Safeguard Adults – Fulfil safeguarding responsibilities by ensuring vulnerable adults are protected from harm and abuse.
- Case Management – Attend and contribute to strategy discussions, case conferences, and key decision-making forums.
- Record Keeping – Maintain accurate, timely records and ensure all documentation complies with legal and regulatory standards.

Applicant Requirements
- Experience – Proven experience working in Adult Social Care, particularly in safeguarding and complex assessments.
- Qualifications – Degree or equivalent qualification in Social Work and registration with Social Work England.
- Compliance – Enhanced DBS check and Right to Work in the UK.
- Skills – Strong understanding of safeguarding protocols, excellent communication, and case management abilities.
