Sanctuary Personnel, an innovative and committed recruitment agency has a job based in Maidenhead for an Adults Social Care Practitioner to work within the Adults and Community Services/Physical Disability and Older People Team.

The salary for this permanent Adults Social Care Practitioner role is £23,625 per annum.


  • Deliver Statutory Services to people who meet the eligibility criteria of the Adult and Community Services Directorate, by supporting the self-assessment of their needs and arranging support in meeting their outcomes and needs especially to enable them to remain within their own home or return to their own home following a hospital admission or other placement
  • Manage an allocated caseload of RBWM residents who may be either within their own homes, or within a hospital setting
  • Work in partnership with them, and with any other multi-disciplinary team involved, in order to achieve positive outcomes that support independent living
  • Facilitate as required the implementation of interventions in order to meet residents identified outcomes and achieve their optimum functioning and independence using 3 conversation model (EST )
  • Ensure the delivery of the SDS process, ensuring that support plans are appropriate to achieve outcomes identified for those who are vulnerable
  • Maximise the extent to which service users take control of the SDS process consistent with ensuring appropriate safeguards against risk
  • Ensure all aspects of safeguarding of vulnerable adults are considered and addressed with supervision and support in accordance with borough policies and protocols
  • Ensure that any identified need for equipment or minor adaptations is carried out under the appropriate guidance in accordance with current processes and policies
  • Ensure any handover of responsibility for subsequent assessments/specialists, assessments and support planning is seamless for the service user
  • Identify individual risk to individuals, families, carers, groups, communities and colleagues. And recommend mitigation and advice
  • Ensure assessment and delivery of services is within the eligibility criteria of the Directorate and within budget constraints
  • Maintain comprehensive and up to date case records that demonstrate good practice and evidence that RBWM is discharging it’s duties in accordance with relevant legislation
  • Provide appropriate advice, support and advocacy to ensure that service users best interests are maintained
  • Manage and be accountable, with supervision and support, for own practice and to take responsibility for own professional development
  • Attend meetings relating to the work of the team or work with individual service users as required
  • Work within the requirements of relevant legislation, the national agenda, and the Policies and Procedures of RBWM as appropriate to the responsibilities of the post
  • Contribute positively to the performance of the team, service and unit, by being aware of, and contributing to achieving key performance targets

Requirements of the Adults Social Care Practitioner:

  • NVQ Level 2,3 and/or 4 or equivalent in Health and Social Care/Promoting Independence
  • Current Social Work England registration
  • A level of education that demonstrates good levels of literacy and ability to conceptualise
  • Experience of working in a Health and Social Care setting
  • Experience of working with older people or adults with a physical disability
  • Takes a proactive approach to meeting the needs of customers
  • Is approachable and helpful when dealing with customers
  • Acts with honesty, integrity and discretion, takes ownership for resolving customer queries
  • Displays drive and energy to achieve results

  • Ability to identify needs and urgent support requirements alongside service users, carers and hospital professionals
  • Knowledge of relevant legislation
  • Ability to represent the Authority, at a multi-agency level, in planning and reviewing services for individual service users
  • Have negotiating skills and the ability to problem solve
  • IT Skills – competent in use of Microsoft Outlook & Word
  • Have good communication skills both written and oral, including report writing
  • Able to work on own initiative without close supervision
  • Ability to organise own workload, work confidently under pressure and meet deadlines
  • Understanding of resource management, including working with finite financial resources and the application of eligibility criteria
  • Understanding of the importance of the Performance Assessment Framework for Social Services and its impact on service delivery
  • Must be able to work in non-discriminatory way, promoting dignity and equality and respecting diversity
  • Ability to represent the Authority, at a multi-agency level, in planning and reviewing services for individual service users
  • Must be committed to working as part of a team to provide high level of service
  • Will hold current drivers licence and have daily use of own car
