What you will do:

• Provide professional social work support to adults with care and support needs, undertaking a range of statutory duties in accordance with relevant legislation.
• Assess the needs of individuals and their carers, planning, implementing, reviewing, and evaluating outcomes to ensure high-quality support.
• Promote principles of choice and control, empowering individuals to make informed decisions about their care and support.
• Adopt a strengths-based approach to help individuals build support networks and achieve their identified goals.
• Manage an autonomous caseload while embracing a multi-agency approach, collaborating effectively with health and care sector colleagues.
• Deliver advice and support to other adult care practitioners, guiding them in their work with clients.
• You will practice in accordance with Social Work England, Professional Capabilities Framework, and Knowledge and Skills Statement for Social Workers in Adult Services.
• You will manage workloads in line with your qualifications and experience while enabling self-directed support for individuals and their carers.
• Undertake risk assessments, ensuring adherence to legal principles of safeguarding and positive risk-taking. Foster innovative use of community resources and technology in your practice.
• Contribute to the development and supervision of social care workers and undertake roles in more specialised areas, such as Approved Mental Health Professional or Best Interest Assessor, as needed.
