Adult Social Worker – Services Transition Team (Adult Services Transition), Medway, Kent
Hourly Rate: £38 | Contract: Ongoing (Initial 3-month contract with potential for extension)
Working Pattern: Part-time (3 days per week) | Location: Hybrid (Medway-based)
Essential Requirement: Access to a car with business insurance cover
Key Responsibilities:

Undertake comprehensive assessments in accordance with the Care Act 2014 and the Mental Capacity Act 2005.

Develop and implement person-centred support plans for young adults aged 18–25 transitioning from Children’s Services.

Work collaboratively within a multi-disciplinary team to ensure cohesive and holistic care delivery.

Maintain accurate, timely, and professional records in line with statutory requirements and best practice standards.

Provide guidance and support to families and carers, facilitating access to appropriate community services.

Engage in team meetings, contributing to service development and continuous improvement.
Candidate Requirements:

Qualified Social Worker with current Social Work England registration.

Demonstrable experience in adult services and transitions work.

Ability to manage caseloads effectively in a hybrid working environment.

Must not have held a permanent role at listed councils within the last 6 months due to MoC restrictions.
