Sanctuary Health is currently looking for a Band 6 Community Psychiatric Nurse to work full-time within an Older People Team based in Bath. The ideal candidates will possess highly developed specialist knowledge of Mental Health Nursing and be registered with the NMC. The pay rate for this Community Psychiatric Nurse role is £30 per hour (LTD Company Equivalent) and is a 2-month contract. Job Duties: Following initial assessment by Primary Care Liaison to provide further high quality comprehensive mental health needs assessment, including risk assessment, for service users. To be responsible for a specific caseload of service users using the integrated CPA process and acting as care co-ordinator. To establish with the service user and carer (where appropriate) an agreed care plan, which includes evidence based interventions to address assessed needs, how outcomes will be measured and agreed timescale for input. To arrange such services as are appropriate to the care plan using the care management process. To regularly evaluate the care plan with the service user and alter as required. To use the principles and processes with the agreed ICPA policy and to act as care co-ordinator when appropriate. To undertake specialist assessments, e.g. Cognitive testing. Requirements of the Community Psychiatric Nurse: Valid NMC registration under Mental Health. Experience working within a similar role would be highly desirable.
