We require a Qualified Social Worker to work as a Care Manager in Cornwall. This is a contract position with an immediate start. The pay rate for this role is £24 Ltd per hour.
 

Duties Include -
-To provide recommendations to the Care Manager on the basis of assessed need.
- To act as a Social Work Practitioner on an ongoing basis to clients.

- To implement Care Planning arrangements.

- To reassess, review and evaluate and monitor care as required.

Requirements -

You must have a relevant Social Work qualification (BA / MA Social Work / DipSW / CCETSW), be GSCC registered and be eligible to work in the UK. It is essential all candidates have 2 years plus post graduation experience in similar Adults Team and also hold a full UK driving licence.
