Qualified Social Worker - Transition Team

We require a Qualified Social Worker to work as an Experienced Team Manager within an Transition Team based in North West London. This is a contract position with an immediate start. The pay rate for this role is £37 per hour.

Duties Include -
- To undertake day to day management of a multi-disciplinary team of staff, providing a service to disabled young people aged 14-19 and their carers.
- To evaluate outcomes for disabled young people and families and to ensure that key performance targets are met.
- To ensure that all statutory responsibilities and duties of the team are met.

Requirements -
You must have a relevant Social Work qualification (BA / MA Social Work / DipSW / CCETSW), be registered with the GSCC, SSSC, Care Council of Wales or NISCC, be eligible to work in the UK.
