Qualified Social Worker to work within an Adults Hospital Team based in Kent with a pay rate of £25-£27 per hour.

Duties Include:

· To take responsibility for & complete assessments.

· To lead on & complete holistic assessments of the social care needs of individuals with regular, structured supervision.

· To lead on identifying & assessing continuing care need

All applicants that want to be considered must have a recognised social work qualification and be GSCC registered with 3+ year’s experience.
